card fraud at the gas pump

Has anyone else experienced this? You look at your banking transactions and see some activity that you don't remember purchasing? Upon further investigation, it appears someone has used your debit card to purchase fuel.

It is too easy to commit fraud at the gas pump because no PIN is needed. I have had this happen twice in the past six months (to the tune of $170 before I noticed the first time). Luckily I have gotten the charges reversed.

But what I would like to know is how do people use my debit card without physically having the card? Do they clone it or something? Other than using cash for everything, is there any way to prevent this from happening?
